Comes with an attachment plate for both sides of the sheath - 2 total- which allows you to swap sides/directions for different IWB, OWB, Scout, upsidedown, left, right, Molle, vest, pack, etc. Also an extra screw will be in the package to switch to Scout carry.
I carry the Stowe (Same exact sheath design) on my hunting pack shoulder strap and it stays right there in my sheath. It never falls out - until I pull it out. I wear it upside down as well, just saying. So, rest easy and don't worry about losing your knife.
Also, the factory sheath ESEE gives you is garbage. Throw it away and get a real one.
Kydex will scratch metal, so if you are bothered by that incredibly, get leather.
