* Enter personalization information in box or in "notes to seller" at check out. Please enter name and address--or any other text (will be printed on 3 or 4 lines depending on length) (Approx. 20 characters/line) *Proofs sent on request*

* If you are purchasing as a gift and would like to include a personalized note for the recipient, please let me know what you would like written on the note.
