After names are pulled online, we "sneak" into the break room to grab the questionnaire for the person that we drew. *ANOTHER OPTION*If you want to manually draw names, you can have everyone complete the questionnaire, fold them & place them all in a box - shake and have everyone pull. You'll just have to make sure they don't pull themselves and if they do - they can switch with someone.
