Instructions are included in the file.
- Select your pay period so the annual premiums can be calculated properly.
- Use the open enrollment paperwork from your employer and transfer the details of each plan (up to 3) to the Inputs tab of the spreadsheet.
- Enter the premium amount per pay period, along with the individual and family deductible and out-of-pocket maximums and any employer contribution (for example for high deductible plans).
- Enter the co-payment and co-insurance amounts per plan, along with the expected visit costs for PCP, specialist and ER visits, along with generic and brand pharmacy costs.
- Enter the number of your prior year visits for each family member as a predictor for next year's costs.
- Switch to the Comparison tab to determine which plan wins and compare expected costs versus maximum possible costs for each plan.
